Dr. Stephen Jurak
is retiring, April 1, after 20+ years of
practice at Comprehensive Pediatrics.
We wish him the best as he moves on to the
next adventures in his life with his family.
Dr. Jurak has enriched the lives of many
patients and families, and we will all miss
him very much.
Dr. Jurak says goodbye

the Westlake office.
We would like to give Dr. Jurak a tangible
remembrance of the patients and families he
has cared for and nurtured. We will be
collecting cards, notes, pictures, a special
story- whatever memories and warm wishes you
would like to share about Dr. Jurak. If you
would like to contribute to his memory book,
please send or bring your “memories” to our
office by March 5, 2015.
Comprehensive Pediatrics welcomes a new
provider! Brigid Strauss, a
Certified Nurse Practitioner, will be
joining our practice March 16, on a full
time basis. Brigid has been working
with us on Saturdays since 2013 and has been
practicing since 1997.
There will be NO
evening hours on March 17. An on call doctor is
when the offices are closed.

We ask that you
arrive 10 minutes early for your
appointment to expedite the check-in
process. This will help us to keep
our schedules on time. We will be requesting a copy
of your drivers license and insurance card
at every visit. Please bring

A charge of $50.00 will be
assessed to your account if you "no show " for an
appointment. This is not covered by
Minors under 18 years of age must be accompanied by a parent
or legal representative to every visit. Consent forms to treat a
minor in the absence of the parent or legal guardian can be found on the
insurance link. Written documentation is required before a minor
can be seen.
YOU DO NOT HAVE YOUR CURRENT INSURANCE CARD WITH YOU AT THE TIME OF YOUR
VISIT, YOU WILL BE REQUIRED TO PAY IN FULL OR RESCHEDULE FOR A TIME WHEN
YOU HAVE YOUR INSURANCE CARD WITH YOU.
If your child is 18 years of age or older,
an authorization to release medical information must be on file in the
chart or we cannot discuss medical information over the phone with the

Registered nurses provide advice
to our patients Monday - Thursday 9a-430p, Friday, 9a-330p. There is
no nurse advice available on Saturday.
Patients are seen in the office by APPOINTMENT ONLY. If
you arrive 15 minutes after your scheduled appointment time, the
appointment will be rescheduled.
Prenatal tours are offered monthly at the Westlake office. Meet one of the
doctors, tour the office and learn valuable information about our

Requests for medication refills will be phoned to your
pharmacy or will be ready for pickup within 24 hours of the request.
Refills are not done on the weekends. A nurse
will contact you to confirm the medication and dosage. Medication cannot
be refilled without this information.
Refills for long term medications require a follow up
every three months and a yearly physical. This includes medications for
asthma and ADD/ADHD.

If you use a mail order pharmacy, we do NOT fax refill
requests or new prescription orders. You need to mail any prescription,
so plan accordingly to receive your medicine on time.
Forms for school, sports, camp etc. requiring an MD signature, can only be filled out if your child has had a physical within the last
year. All forms must include the child's name and date of birth.
If the form is for medication, please include the medication
requested on the form, failure to provide this information will
delay completion of the form. Please allow 5-7 days if you are dropping off a form to be filled
